Statement of Purpose
Under the direct supervision of the Health Information Management Supervisor the Health Information Management clerk has the responsibility of filing, logging, retrieving, and delivering medical records as required. Requires the ability to maintain confidentiality of sensitive information.
Essential Functions, Knowledge, Skills and Abilities
- Routes and handles provider’s bins in coordination with back office staff.
- Copies and assists patients and authorized agencies with requests for medical records. Prepares and mails invoices for requested medical records to authorized agencies.
- Charts must be picked up each evening and locked in the medical records department before leaving for the day.
- Responsible for printing out provider schedules for the following business day, pull the charts, for providers who are not on HER.
- Deliver, transport, sort and file charts.
- Responsible for pulling charts as needed throughout the day for patient appointments and special requests.
- Responsible for scanning all medical reports including lab, x-ray results, dictation and correspondence from the courier, hospitals, etc. into the patients’ charts daily.
- Purge obsolete records and file them in storage.
- Work with medical assistants and other staff to maintain patient charts.
- Merge duplicate charts to one complete patient medical record.
- Responsible for handling incoming calls related to medical records.
- Prepares and maintains logs of all incoming/outgoing medical records requests.
- Monitors and distributes faxes.
- Participates in professional development activities.
- Maintains strictest confidentiality.

- Communicates openly and consistently with Supervisor(s) and all staff.
- Other duties as assigned related to medical records department
- Assist with morning huddle by retrieving consultations, labs or X-ray reports as needed.
- Knowledge of medical records filing systems both alphabetic and numerical.
- Develops trust and teamwork with staff and patients.
- Ability to read, understand, and follow oral and written instruction.
- Ability to maintain confidentiality of sensitive information.
- Ability to maintain patient records by filing and/or scanning into EHR.
- Skill to write patient information legibly.
- Knowledge of clinic policy and procedures related to this position.
- Knowledge of grammar, spelling and punctuation.
- Skill in operating a computer, telephone, and photocopy machine.
- Ability to sort and scan materials correctly into patient Electronic Health Records.
- Ability to establish and maintain effective working relationships with patients, staff and the public.
- Ability to react calmly and effectively in emergency situations.
Required Education and Experience
Minimum Education: High school graduate or GED equivalent required.
- Knowledge of medical records filing system.
- Medical Assisting certificate from a credentialed school.
- CPR certification preferred.
Preferred Education and Experience
- Bilingual in English and Spanish is preferred.
- One year medical records experience in an office or clinic setting is preferred.

Work Environment
Office environment with controlled temperature.
Position Type/Expected Hours of Work
This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 5:00 p.m. Occasional evening and weekend work may be required as job duties demand.
Travel
Little to no travel is expected for this position. Typical example of travel would be driving to Altura’s various clinic locations.
