JOB TITLE: Speech-Language Pathologists
RESPONSIBLE TO: DPS
DIRECTLY SUPERVISES AND EVALUATES: N/A
DESCRIPTION: Responsible for assessments and treatments of speech and language disorders, including development, mental, and organic involvement.
QUALIFICATIONS / KNOWLEDGE
Licensed and currently registered as a Speech-Language Pathologist pursuant to Article 159 of the New York State Education Law.
Formal education preferred:
Master's degree with a major in speech-language pathology. Holds a certificate of clinical competence and licensure granted by the American Speech and Hearing Association.
Previous experience required:
Minimum of one year of experience in speech-language pathology.
Knowledge and Abilities
1. Able to support the concept of family-centered care.
2. Able to read, write and speak the English language.
3. Demonstrated knowledge and skills necessary to provide care to and communicate with the patient population.
4. Able to work as a member of a team.
5. Able to assess data reflecting the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their specific needs.
JOB DUTIES / RESPONSIBILITIES 60%
ESSENTIAL DUTIES
1. Provides services in speech pathology in accordance with the physician's plan of treatment. Accepts only patients referred by the Rehabilitation Supervisor or designee. Demonstrates competency in the skills required by the agency on at least a bi-annual basis.
2. Assists the physician in assessing the patient's level of functioning and the development (and
Revision, as necessary) of the plan of care by applying appropriate tests for speech, hearing and language disorders.
3. Records and reports the patient's reaction to treatment and any changes in the patient's condition to the Nurse Coordinator/Clinician/Case Manager or Rehabilitation Supervisor.
4. Instructs and advises the patient, family and other health team personnel in the phases of speech pathology in which they may assist the patient.
5. Recommends hearing tests by audiologist when necessary. Checks hearing aides.
6. Assesses the patient's communication, oral-motor integrity, cognitive status, memory and problem solving activities, hearing and speech fluency and quality.
7. Cooperates and consults with appropriate staff to provide staff education when requested and arranged by the Rehabilitation Manager
8. Evaluates, and regularly re-evaluates the speech pathology needs of the patient; initiates, develops, implements and makes necessary revisions to the patient's plan of care. Assesses the patient's continual care needs. Includes all problems identified in the assessment in the plan of care or documents rationale for not doing so.
9. Initiates diagnostic, preventive and rehabilitative procedures as appropriate to the patient's
care and safety. Makes referrals to other disciplines as indicated by the needs of the patient or documents rationale for not doing so.
10. Observes signs and symptoms and reports changes in the patient condition to the physician and/or other appropriate health professionals as often as needed.
11. Teaches, supervises and counsels the patient and family regarding home procedures and
other care needs as appropriate to the patient's condition. Utilizes agency educational material as appropriate.
12. Coordinates the total plan of care and maintains continuity of patient care by liaising with other health professions. Attends patient care conferences. Initiates patient care conferences for complex and/or multi disciplinary patients when needed or helpful to
ensure good coordination of care.
13. Develops, prepares and maintains individualizes patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation within 24 hours of visits.
Participates in the quality improvement program of the agency.
15. Keeps abreast of speech pathology trends and knowledge.
16. Participates in service programs.
17. Participates in the Quality Improvement Committee.
