SUMMARY
Under the direction of the Residential Supervisor, the DSP staff supports the individuals to achieve the highest level of independence and quality of life. DSPs will provide assistance with activities of daily living, appropriate social skills, medical and behavioral support plans, self-medication, medical treatments, and community integration. DSP will assist individuals to maintain their homes or apartments so they are clean, safe, and free from hazards. DSP will maintain individual’s satisfaction of services.

ESSENTIAL DUTIES AND RESPONSIBILITIES
- Assists individuals in maintaining a safe and healthy lifestyle, and monitor for changes in their general health, appearance, behavior and activity. Note in individual’s file and communicate changes to QIDP/director/family/caregivers or physicians, if needed.
- Monitor and maintain a clean, safe, and secure environment for individuals at all times.
- Support individuals in maintaining general good health by following up on all physician recommendations.
- Assist individuals in administering or monitoring the medication process and provide proper documentation.
- Ensure individuals are free from abuse, neglect and exploitation and are treated with dignity and respect at all times.
- Ensures implementation of Person Centered Plans and documents outcomes accurately and in a timely fashion.
- Provide training and assistance to individuals in healthy dietary choices.
- Assist individuals in personal hygiene tasks as needed.
- Plan and direct individuals in daily activities.
- Assist individuals in performing housekeeping duties such as vacuuming, shopping, laundry and mopping.
- Complete all required paperwork and documentation to meet Opportunity Village’s and DRC requirements.
- If required, safely transport individuals to and from community outings, work, personal appointments, and ensure vehicles are maintained according to Opportunity Village’s vehicle policies.
- Performance of all job duties should be professional at all times and consistent with Opportunity Village’s culture, mission, values, philosophy, and policies and procedures.
- Attends all mandatory trainings and staff meetings as assigned.
- Act as an advocate for individuals in community relations.
- Maintains and adheres to ethics of confidentiality and HIPAA.
- Provide support with individuals emotional issues and provide support and solutions.
- Report any incidents, safety issues and workplace injuries immediately.
SUPERVISORY RESPONSIBILITIES
- This position does not have any supervisor responsibilities
