Summary:
This position is responsible for coordinating on and off campus appointments and transports. Providing professional nursing care during appointments and transports in accordance with all applicable state licensure and regulatory requirements. Provides clinical oversight of CNAs.
Essential Functions (in addition to RN essential functions):
- Schedule virtual and non-virtual medical and health related appointments.
- Arrange appropriate transportation and accompany residents to non-virtual medical and health related appointments.
- May participate in virtual appointments as available.
- Coordinate pre lab draws with nursing. If nurse unable to draw, Transport RN to draw. If unable, transport for draw in a timely manner.
- Provide appropriate medical information to health care professionals and collaborate care.
- Complete an appointment summary note and file provider summaries in electronic medical record (EMR).
- Provide verbal report to primary nurse as appropriate.
- Communicate with parent/guardian as appropriate.
- Assesses ways to improve transports with Director of Nursing (e.g., community activities).
- Other duties as assigned.

Required Qualifications
Degree from accredited School of Nursing
Current Registered Nursing License with WA State Department of Health
Must pass a State and Federal Background Check, including fingerprints
Valid State Driver’s License
Must maintain current vehicle insurance
Current Healthcare Provider BLS/CPR
Current Food Handler’s Permit or within 30 days of hire
Proof of required vaccinations
DDA Developmental Disabilities Specialty Training within one year of hire
Must acquire and provide proof of 30 hours of CEU’s per year
Preferred Qualifications
3-5 years vent experience
3-5 years acute/ICU experience
Lab draws
Bachelor of Science in Nursing
Work Environment
This role operates in a home health care facility and includes going with residents to outside medical and health related appointments. The position requires flexibility and willingness to work nights, weekends, and holidays. The environment may include exposure to communicable diseases, bodily fluids, and other potential hazards. Must wear Personal Protective Equipment (PPE) as required and follow strict guidelines to protect self and residents from infectious disease; use proper lifting techniques and pursue help as needed.
Physical and Sensory Demands
Position requires full range of motion. Must be able to walk, bend, stretch, lift and stand. Must be able to grasp and use fine hand coordination. Must be able to distinguish between smells, must be able to hear audible sounds, must be able to lift 40 lbs., must be able to operate a car safely.
