The Prevention Case Manager is responsible for the primary program they are assigned to as well as being an active team member in Prevention Services. Prevention Services programming includes outreach to at-risk families, youth and/or other vulnerable populations to prevent them from becoming involved in the child welfare and/or juvenile justice systems while strengthening their stability, safety, connections and resources.
Responsibilities include:
- Meet with and provide case management and/or therapeutic support to youth and families who are referred for Prevention Services programs including obtaining all relevant individual and family information (releases of information, HIPPA, background, etc.)
- Deliver support services to youth and their families as assigned to ensure that target program goals are being met or exceeded.
- Complete initial and ongoing behavioral assessments and develop individualized service plans for youth and their families with appropriate, measurable goals.
- Monitor youth and families involved in the Prevention programs through consistent face-to-face and collateral contacts including schools, mental health providers, employers, courts, community partners, etc.
- Maintain required records including client files, contact logs, court reports, etc. in a timely manner consistent with OCS policies and procedures.
- Assess and refer Prevention program youth and their families to appropriate community service providers and complete necessary referral paperwork.
Required Qualifications
- Master's degree in a social services field (starting salary will reflect experience)
- Current BRSB license or ability to become licensed in the state of Kansas
- Able to pass KBI background check
- Able to pass a CANIS check
- Possess a valid Kansas driver's license with driving record that satisfies the agency's insurance
- At least one year experience working with youth, families, and other vulnerable populations
Preferred Qualifications
- Experience working with the Child in Need of Care/Juvenile Justice systems
- Experience with providing therapeutic services to youth and families
