Job Summary: Demonstrates proficient skills and knowledge in patient assessment, planning, interventions, and evaluation. Effectively communicates with physician to execute patient plan of care. Coordinates and supervises the delivery of patient care with all interdisciplinary team members. Supervises the licensed practical nurse and hospice aide. Maintains compliance with policy, procedures, State and Federal regulations, State Board of Nursing, and accreditation standards.
Qualifications/ Educational Requirements:
1. Registered Nurse (RN) with a current license in the State of Utah or compact licensure.
2. Two years clinical nursing experience preferred.
3. Excellent communication skills.
6. Excellent interpersonal skills.
8. Professional demeanor and appearance.
9. Current CPR certification.
Responsibilities/essential functions: Must be able to perform the following essential job functions with or without reasonable accommodations.
1. Assess, plan, implement and evaluate the patient plan of care.
2. Performs initial and comprehensive assessments.
3. Develops a plan of care centered on issues, goals and interventions based on patients’ goals for care.
4. Initiate’s patient plan of care under the direction and orders of the physician.
5. Monitor and evaluate the plan of care in coordination with the interdisciplinary team.
6. Effectively manage patient symptoms in coordination with the physician.
7. Educate patients, families, and caregivers on techniques for in home care and end of life care.
8. Presents patient clinical status and progression of decline in IDT meetings.
9. Timely notifies physician, nurse supervisor, and other interdisciplinary team members of patients’ status, changes in condition and other relevant information.
10. Supervise the licensed practical nurse and effectively delegates assignment.
11. Trains and supervise the hospice aide care.
12. Document at point of care, documentation is accurate and reflects patient condition and decline.
13. Completes education, participates in education in-services, continuing education programs and seminars.
14. Provides education to hospice employees and community partners.
15. Maintains compliance with Medicare Conditions of Participation, State and Federal laws, Utah Nurse Practice Act, CHAP accreditation standards, and hospice policy and procedures.
