The Program Coordinator, QIDP trains and mentors Direct Care Staff; develops implementation plans and programs for participants; facilitates contact with participants' family members; assists participants in maintaining well-managed households. Works in a team environment with other Program Coordinators and staff. Work schedule consists of some afternoon/evening hours and some weekend hours.
Principal Duties and Responsibilities
- Work as a team to coordinate, support, and work with other HISway Program Coordinators or Administrative staff to ensure continuity of care for all Participants receiving services from HISway, LLC.
- Being responsible for Service Plan Developement and implementation for individuals with developmental disabilities.
- Knowing and enforcing IDAPA policy as well as agency policies.
- Complete ISP/ Status reviews and send to the ISP team within regulated time frame.
- Manage staff, participant and guardian concerns as they arise.
- Work with House Supervisors/Coordinators to create staff schedule based on current ISP.
- Program coordinators will have a rotating on call schedule to include overnight/weekend hours.
Supervision Received:
Program Coordinators will also receive guidance from the Administrator. Supervision will be a general oversight based on compliance and an as needed basis. It is the employees’ role as Program Coordinator to ensure all duties and responsibilities are being met.
Supervision Exercised:.
- Program Coordinators will ensure House Leads and Direct Care Staff are properly trained and supported in their roles. The Training Officer will assist Program Coordinator(s) in training/hiring//developing and evaluating of Direct Care Staff.
Qualifications & Skills:
REQUIRED:
- Be at least 18 years of age.
- Possess a Bachelor’s Degree in a Human Service field
- Posess a Qualified Intellectual Disabilities Professional (QIDP) qualification
- Minimum of 1-year supervised experience with population served
- Ability to pass a criminal history background check
- Ability to obtain and maintain current CPR/First Aid
- Ability to obtain assistance with Medications for Unlicensed Personnel (UAP) card
- Possess and maintain automobile insurance at all times during employment
