New Hope Gracious Personal Care’s goal is to enhance the lives of seniors by providing a place that feels like home. The Resident Care Director is a supervisory position that ensures the care staff is properly trained, and provides the oversight of the care staff and clinical operations to make certain the highest quality of care is provided to the residents residing in the home.
Essential Functions, Duties, and Responsibilities include but are not limited to:
· Help to ensure the facility remains in regulatory compliance with the PA Department of Health and Humans Services 55 Pa. Code Chapter 2600.
· Hire, supervise, train, evaluate, discipline, and provided needed information to subordinates.
· Take an essential role in the admission process by assisting the Administrator with the assessment process and regulatory paperwork.
· Evaluate residents care needs and determine the needed level of care upon admission.
· Maintain a clear medical record of resident changes in condition, requests, and responses.
· Work with staff to appropriately update the medical chart, DME, and assessment plan as needed.
· Observe residents’ health status and address within scope of practice.
· Record and notify Administrator, physician pharmacist, and family of any significant changes to a resident’s health status, drug reaction, treatments, or significant incident.
· Ensure appropriate supplies are ordered and maintained to meet resident needs.
· Oversee the scheduling, staffing, and attendance of Med Techs and Caregivers.
· Monitor colleagues in medication administration and management with audits and retraining.
· Participate in meetings as assigned by the Administrator.
· Maintain professional accountability for personal nursing practice and care delegated to non-licensed associates.
· Administer medications as facility needs occur.
· Assume the role of the Administrator in their absence.
Qualifications/ Education:
· LPN license in the state of PA
· Knowledge of Geriatrics, common medical concerns, and related disorders.
· Experience in Personal Care preferable or other medical setting.
· Willingness to work as part of a collaborative team.
