QIDP/ Habilitation Service Coordinator:

QIDP/ Habilitation Service Coordinator - Responsible for the oversight and coordination of planned supports for people receiving residential habilitation and other services. The role ensures that quality services are developed, implemented, and documented. The role collaborates with other clinical professionals while guiding direct support staff in the delivery of person-centered services and supports in residential and day program settings.
Specifically, as a QIDP/ Habilitation Service Coordinator you will:
- Oversee and monitor service delivery documentation and verify that services meet Medicaid compliance needs for billing purpose.
- Observe and assess maladaptive behavior
- Develop and implement treatment plans to address behavioral issues
- Work directly with and train direct support staff on the implementation of behavior plans
- Monitor and assess behavioral data
- Coordinate with the Director of Clinical Services for specialized services such as speech therapy, physical therapy, occupational therapy, and counseling
- Present to Human Rights Committee
- Meet regularly with House Supervisors and Residential Nurses
- Regularly review residential habilitation service documentation and monthly billing, making corrections as needed
- Routinely participate in evaluating care and services, fire and environmental safety
- In the absence of the House Supervisor, ensure timely implementation and documentation of fire drills and assist with other supervisory duties
- Conduct on-site visits during program times to ensure the proper implementation and documentation of the program plans, to monitor care, and to provide guidance and support to direct support staff
- Liaison with families and other natural supports
Minimum Qualifications:
- The QIDP/ Habilitation Services Coordinator must meet QIDP qualifications
- A Master's Degree in a major related to the field is preferred
- Must have experience working with people with autism and other developmental disabilities in a residential setting is preferred
- ABA certification a plus
- Consideration will be given to those candidates who do not hold a Master’s degree, but meet QIDP qualifications and possess experience equal to the duties of the position
- Must have an understanding of OPWDD regulatory requirements as they relate to certified settings
- A driver’s license and a reliable vehicle are also required

Pay: $28.00 - $30.00 per hour
