- Assists in patient’s adjustment to their illness/condition and adherence to the treatment plan, as well as ensuring that they are aware of, and have access to, appropriate community resources. Includes patients/caregivers in the involvement or formulation and implementation of the patient's plan of care, and interdisciplinary collaboration will be employed to ensure the greatest continuity of care.

- Minimum Education:
- Required:
- Master’s Degree in Social Work
- Preferred:
Minimum Years of Experience (Amount, Type and Variation):
- Required:
- Three years social work experience, preferably with older adult population; experience with community agencies helpful.
- Preferred:
- Three years social work experience, preferably in Home Health.
License, Registry or Certification:
- Required:
- New Jersey Licensure for Social Work, LSW or LCSW.
- Valid NJ driver’s license.
- Preferred:
Knowledge, Skills and/or Abilities:
- Required:
- Strong clinical assessment skills and the knowledge and ability to respond to routine social work related inquiries.
- Knowledge of local community resources.
