Title: Social Worker
Schedule: M – F, 8AM – 5PM
Location: Hybrid-(this position will require the Social Worker to work remotely 75% of the time and 25% of the time on the field in San Diego).
Compensation: $32/hr. - $41/hr.

Job Description
In collaboration with the care team, the Social Worker is responsible for providing a range of social work services including psychosocial assessments, interventions, crisis intervention, short term case management, providing information and connecting resources for our patients and/or families to meet the needs of the members, family members and caregivers.
Qualifications
Minimum Education: Bachelor’s degree in Social Work or related field required. Master’s degree in Social Work or related field preferred. MSW or LCSW preferred.

- Expereince in ambulatory case management preferably is a medical group or health plan setting.
- Experience in working with diverse population with a wide range of problems.
- Expereince in working with underserved population preferred.
- Knowledgeable in HMO, Medi-Cal and/or health services experience.
- Remote with 25% field responsibilities in San Diego
Minimum Experience: Two (2) years’ experience in ambulatory case management preferably in a medical group or health plan setting required. Experience in working with a diverse population with a wide range of problems (i.e., death and dying, substance abuse, disability, chronic illness, etc.) required. Bilingual language proficiency preferred. Experience in working with the underserved population preferred. Ability to prioritize and multi-task. Excellent verbal, written and computer skills. Ability to perform psychosocial assessments and develop and implement care plans. Ability to exercise objectivity and good judgment relating to difficult and emotionally charged situations. Ability to interface effectively with patients, providers, management, and employees of the organizations as well as facilities and agencies in the community. Working with the needs of seniors or persons with disabilities (SPD) in a customer/member service capacity, HMO, Medi-Cal and/or health services experience preferred. Maintain professional attitude when working with internal and external customers.
Responsibilities
- Maintains a caseload of members with primary focus on social determinants of health, psychosocial and behavioral needs.
- Conducts telephonic and/or in-person Social Work assessment and interventions to include, but not limited to evaluation of basic needs: clothing, housing/homeless prevention, limited financial resources, referrals, food, safety in the home, transportation, utilities assistance.
- Collaborates with the health plan, PCP, providers, and health care team to ensure timely communication and coordination of members’ needs.
- Identifies the need for and facilitates appropriate community resources including referrals to Long Term Support Services (LTSS), Behavioral Health.
- Guides members in understanding and accessing the health plan benefits they are entitled to.
- Acts as a consultant to patients requiring information on Advanced Directives and/or end of life planning.
- Provides counseling and crisis interventions to patients and/or families over issues related to psychosocial issues.
- Coordinate, reports, and/or provide guidance to Outpatient team in reporting suspected cases of abuse/neglect when identified.
- Processes incoming referrals per set processes
- Maintain and ensure confidentiality of patient information
- Attend and contributes to team conferences regarding care to/for specific member.
- Maintains timely and accurate documentation of assigned member’s case, care plan, interventions, and interdisciplinary care team within the care management system.
- Maintain/update a library of community and county-based resources relevant to the needs of the population.
- Provides in-services to staff as applicable on social service issues and community resources.
- Participation in Social work-related projects and initiatives as identified.
- Maintains compliance with established departmental productivity guidelines.
- Complete required Health plan and company trainings
- Maintain internal department trackers and reports.
- Other projects and duties as assigned

Pay: $32.00 - $41.00 per hour
