Job Summary:
We are seeking a highly skilled and compassionate Registered Nurse to join our Valley Hospice On-Call team. As a Registered Nurse, you will provide, after hours, direct patient care, administer medications, and assist with various medical procedures. Position works from home until dispatched, by Triage RN, to patients home/nursing facility.

Position Scope:
The Registered Nurse, under the direction of the Patient Care Manager, provides skilled nursing care to patients in an on-call setting under the direction of the triage RN. Implements and evaluates nursing care through systematic assessment of patient/family needs.

I. Essential Functions/Performance Factors
- Provides services in accordance with organizational policies and procedures and regulations that define scope of practice. Primary responsibility is to provide skilled nursing care in the on-call setting.
o Using the NAR (Nursing Assessment Record) or Assessment as a tool or, assess patient’s physical condition with each visit & document findings.
o Complete pain assessment thoroughly with each visit.
o Identify other symptoms and treat appropriately.
- Participates in on-call rotation as applicable.
- Provides skilled nursing care to patients on the Home and Facility teams when required to provide consistent optimal patient care according to staffing needs in accordance with organizational policies and procedures.
· Participates in assessing, implementing and evaluating the nursing care plan according to the IDT Care Plan.
o Implements the care plan related to identified patient/family needs.
o Evaluates the effectiveness of nursing interventions, and make appropriate changes to the care plan.
o Attends team meetings, as appropriate, and participate in the development of the plan of care.
· Provide education to patient’s/families about palliative care, infection control and issues related to death and dying.
Assess patient/family educational needs with each visit and respond with appropriate teaching.
o Evaluate patient/family response to teaching and change approach, as necessary, to meet needs.
o Use hospice teaching tools appropriately.
· Documents care provided in an accurate, complete and timely manner.
o Complete documentation of each nursing visit within 24 hours of visit.
o Complete the NAR or Cerner documentation accurately.
o Write nursing notes that reflect the patient’s terminal illness according to NHO guidelines, Local Medical Review Policies and the care plan.
o Sign all entries with full signature & credentials.
o Provide care/medications in accordance with a physician’s order.
o Chart to reflect the nurse’s role as case manager.
o Document change in status accurately and complete this documentation within 24 hours.
· Uses agency resources wisely.
· Participates in performance improvement activities.
Job Type: Full-time
Pay: $70,000.00 - $80,000.00 per year
