In this role, the supervisor of Case Management in Special needs will provide leadership and oversight for the team of case management who work with individuals with special needs. This position requires office attendance. Below are some responsibilities but also not limited to them.
Supervisor of Case Management:
Reports to the Director of Case Management.
- Provide direct supervision of case management team, including RN Case managers, LPN Quality Reviewers, and Patient Care Liaisons.
- Assigns case load to case managers.
- Oversee case management team performance and work efficiency for timely completion of assigned tasks.
- Conducts yearly performance evaluations and provide constructive feedback.
- Provides mentoring to case management team to foster professional development & adherence to regulations.
- Covers tasks for Case Managers when needed such as short staff or vacation.
- Reviews team vacation requests & develop and implement staffing plans for vacation coverage.
Qualifications:
- Bachelors in Nursing
- Experience in homecare case management
- Strong leadership skills and ability to multitask
Job Type: Full-time
Pay: $110,000.00 - $115,000.00 per year
