Responsibilities:
- Patient Assessments: Conduct thorough assessments to understand patients' needs and develop tailored care plans.
- Symptom Management: Skillfully manage pain and symptoms to enhance patients' quality of life and provide relief.
- Emotional Support: Offer compassionate support to patients and families, fostering open communication and understanding.
- Education: Provide valuable information to empower patients and families in making informed decisions about their care.
- Collaborative Care: Work seamlessly with interdisciplinary teams to ensure holistic and coordinated care for patients.
- Documentation: Maintain accurate records of patient assessments, care plans, and interventions in compliance with hospice regulations.
- Family Liaison: Serve as a liaison between patients, families, and the healthcare team, promoting unity and understanding throughout the hospice journey.
Qualifications:
- RN Licensure: Active registered nurse license, demonstrating proficiency and adherence to nursing standards.
- End-of-Life Expertise: Experience working at a hospice agency is required!
- Compassionate Approach: Innate empathy and a genuine passion for supporting patients and families during the delicate end-of-life process.
- Communication Skills: Excellent verbal and written communication skills to effectively connect with patients, families, and interdisciplinary teams.
- Clinical Competence: Strong clinical skills and critical thinking abilities to provide skilled nursing care to patients with diverse needs.
- Cultural Sensitivity: Respect for diverse cultural and religious backgrounds, ensuring care that aligns with individual values and beliefs.
- Adaptability: Flexibility to navigate dynamic and emotionally charged situations with sensitivity and grace.
