RN Community Living Centers (CLC)
WJM Professionals is DESPERATELY recruiting for Community Living Centers (CLC) RNs for the VAMC in Richmond, VA.
The role of a Community Living Centers CLC RN is rewarding and impactful, knowing you are providing care and support to veterans.
It is a place where Veterans can receive the nursing home level of care, including help with daily living activities and skilled nursing and medical care.
THE RESPONSIBILITIES OF A CLC RN ARE:
· Performing assessments.
· Administer medications and treatments.
· Monitor vital signs and manage acute and chronic conditions. Provide wound care, manage catheters, and assist with activities of daily living (ADLs).
· Provide resident and family education.
· Manage admissions, discharges, and transfers.
· Conduct comprehensive assessments of residents’ physical, emotional, and psychosocial needs.
· Develop individualized care plans based on assessment findings.
· Provide wound care, manage catheters, and assist with activities of daily living (ADLs).
· Work closely with interdisciplinary teams, including physicians, social workers, and therapists.
· Educate residents and families about health conditions, medications, and self-care.
· Communicate effectively with residents, families, and caregivers.
· Offer emotional support and address concerns.
· Maintain accurate and timely electronic health records.
· Ensure compliance with regulations and standards.
REQUIREMENTS:
· Must be licensed as a registered nurse
· Must possess an associate degree.
· BLS certification required.
· Must have experience in geriatric or long-term care nursing.
· Maintains expertise in skilled nursing care, rehabilitation, restorative care, long-term care, and hospice and palliative care.
· Working with elderly and disabled Veterans requires empathy, patience, and understanding.
· Possess knowledge of the Center for Medicaid/Medicare Services Federal Regulations for Long-Term Care.
· Proficiency in clinical skills such as wound care, medication administration, and assessment.
· Proficiency in dementia care and managing complex medical conditions.

Currently, VAMC needs both day and evening shifts with the understanding. These positions work every other weekend and some holidays.
