POSITION 1 - SUMMARY for Social Worker :
Provides social work services to patients, and their families, in both the HomeCare and Hospice programs. Will assess and treat emotional needs related to illness. Will participate as a member of multi-disciplinary team and may assist patients in treatment planning, discharge planning, community resource needs, advanced medical directives as well as performing various assessments/screens. Master's degree of social work required. Experience in HomeCare and Hospice preferred but not required.
***This is primarily a work from office position but there will be some local travel to patients residence required
QUALIFICATIONS:
· Masters degree in Social Work
· One year experience in a health care setting desired
· Current CPR certification
· Able to provide own transportation to patient’s homes
· Satisfactory references
· Positive physical and mental health
WORK CONDITIONS:
· Requires driving/travel to other sites, including in inclement weather.
· Occasional exposure to infectious and contagious entities.
· Occasional exposure to unsafe/unsanitary conditions in patient homes.

DUTIES & RESPONSIBILITIES:
· Responds to referrals of social work services from HomeCare & Hospice.
· Assess psychosocial needs of patients and/or families referred for services by reviewing the medical record, interviewing the patient and/or family and consulting with other members of the interdisciplinary HomeCare & Hospice team.
· Develop psychosocial treatment plan based on patient need, reimbursement guidelines and regulations.
· Participate in interdisciplinary case conferences to discuss severity of psychosocial problems impeding patient progress and promote understanding of significant psychosocial factors impacting on health problems.
· Contact attending physician as indicated to inform of progress and/or receive orders to modify the treatment plan as indicated to address changes in patient status.
· Cooperate with HomeCare & Hospice and interdepartmental services, medical care and community resources in executing the patient/family plan of care.
· Provide advisory and consultative services to HomeCare & Hospice professional staff as indicated.
· Provide formal and informal training regarding communication, conflict resolution, the emotional, social and environmental factors affecting the health and well-being of patients and their families.
· Document the patient/family progress toward identified goals response to treatment plan and assessment findings in patient medical record adhering to documentation guidelines/protocols.
· Complete other HomeCare & Hospice records and reports in an accurate and timely manner.
· Demonstrate ability to organize and manage patient referral activities.
· Participate in and support the HomeCare & Hospice Quality Improvement Program and Utilization Review activities.
· Comply with quality improvement standards, policies and procedures of HomeCare & Hospice.
· Adhere to the NASW Code of Ethics and standards of the profession and HomeCare and Hospice policies.
· Participate in HomeCare & Hospice meetings, activities, programs, in-services and continuing education classes.
· Maintain and increase knowledge base and clinical competence through attendance and participation in other professional development opportunities.
POSITION 2 - SUMMARY: The Behavioral Health Therapist
functions as a core member of the Integrated Care team within Hospital and Clinic. Responsible for coordinating and supporting mental health and behavioral health care within Family Practice.
QUALIFICATIONS:
· One of the following:
· Master’s Degree in Social Work, with a current Michigan license as a Licensed Social Worker (LMSW or LLMSW), or:
· Doctoral degree in Psychology, with a minimum of two years’ experience as a therapist and current license to practice in Michigan as a Clinical Psychologist.
· Experience working with patients who have co-occurring mental health, substance abuse, and physical health problems.
· Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health clinicians.
· Willingness to work with diverse populations
· Heartsaver CPR Certified (obtain upon employment)
DUTIES AND RESPONSIBILITIES:
· Maintain clinical caseload agreed upon by clinic director and CEO. This includes providing quality patient assessments to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment and or life situation.
· Develop individualized treatment plans based on patients’ preferences, values, goals, and needs.
· Utilize evidence-based methods to provide group and individual treatment to patients and families.
· Collaborates with community leaders on projects/initiatives to further develop behavioral health resources in the local community
· Process referrals to psychiatric providers or other practitioners for psychiatric testing or other mental health or behavioral health care needs.
· Provide consultation to and coordinate care of patients with Family Practice primary care staff.
· Complete all documentation as needed and required: including assessments, treatment plans, progress notes, discharge summaries etc.
· Maintain confidentiality in compliance with HIPAA and RHC regulations.
